Welcome to our Medical Records Request page. Here, individuals and authorized outside parties can easily submit requests for access to medical records. Access
Patient Access: Patients have the right to access their own medical records. Requests for access should be made in writing using the designated forms provided.
Third-Party Access: Requests from third parties (e.g., insurance companies, legal representatives) must include a valid authorization from the patient or their legal representative.
Request Submission
All requests for medical records must be submitted in writing using the designated forms provided. The form should be completed in full.
Response Time
The organization will respond to requests for access to medical records within 30 days from receipt of the completed request form.

If you are the guardian, Executor of the Estate or Power of Attorney for the patient, you must submit a copy of the appropriate legal documentation, which proves authority to act on behalf of the patient. This must accompany the Medical Record Release form. These documents can be found on the left side of the page, under our contact information.
Subpoenas
CASA-Trinity is required by law to respond to subpoenas. Subpoenas for records can be accompanied by a valid release of information OR a good cause court order.
